Your contact request
The form asks for your name, telephone and email, with your agreement to be contacted about the request. When the form connection is active, your request is stored by the website and an email notification is sent to hmhaartransplantatie@gmail.com. We start on WhatsApp and arrange further contact together. Language, source page, request date and consent version help us follow up. The form does not request medical details or photographs. A contact request does not subscribe you to a newsletter.
Clinical photos and publication are separate choices
You separately choose whether photographs may be used for planning and your secure clinical record, including a no-photos option. This never automatically authorises publication. Publication permission is optional; declining or leaving it blank does not affect your treatment or price. Consent specifies the exact files, purposes, recognisability and expiry date. Name and voice are not published; a scalp can still be identifiable. You may withdraw through HM: new use stops and controlled publications are removed where feasible. Third-party copies may not be recoverable. Another brand requires a fresh choice.
Website, question helper and external services
This website uses no advertising pixels. The question guide states whether live AI is active. Without it, questions stay in your browser and prepared answers are shown. When AI is active, your question and recent conversation context are sent to OpenAI to generate a reply. We do not save a chat record and ask the API not to store responses; OpenAI processing and any security logs are governed by its terms. Do not share medical details here. Hosting processes technical access information. Form and AI abuse prevention uses a temporary hashed network identifier. Social updates are retrieved through our server. WhatsApp, Instagram and Google Maps open when you select their links and their privacy terms apply. An email service is used for active form notifications.
Access, corrections and your choices
You can ask what information we process and request access or correction. Where applicable, you can also request erasure, restriction or portability, or object. Consent can be withdrawn for processing based on it. Contact us about the use or retention of your request, or to ask for no further contact. You may lodge a complaint with the Dutch Autoriteit Persoonsgegevens.
